CCI, Inc. may provide assistance with Transportation through reimbursement of public transportation, mileage, or rideshare when a person is working and needs transportation to and from their job, or to access the community, and all other options have been explored. The Transportation service is offered by CCI as an Organized Healthcare Delivery System, and does not assume responsibility for transportation with outside parties such as rideshare, public transportation, or any other form of travel reimbursed for paid for by this service.

You or your CCS may send referral information to ccireferrals1@gmail.com. Please send the most recent Person-Centered Plan, Health Risk Screening Tool and best point of contact for the individual applying for services.
Schedule a “meet and greet” with the CCI, Inc. team, learn about our services and a program that would be right for you. Your CCS is welcome to join the meet and greet too!
Once you’ve chosen to work with CCI, Inc. we will work closely with your team to develop a schedule, outcomes and goals and service plans to meet your needs. We will work with your CCS to get you started with services as soon as possible!
